Chelsea midfielder Lewis Baker is believed to be close to joining Championship side Middlesbrough on a two-year loan deal.

Middlesbrough have requested to sign midfielder Lewis Baker on a two-year loan from Chelsea, according to reports.

The England Under-21 international has spent 12 years on the books of the Blues after joining them from Luton Town as a youngster.

However, he is yet to play in a Premier League game for Chelsea, his only senior appearance for the West Londoners coming against Derby County in the FA Cup in January 2014.

According to the Daily Mail, the 22-year-old is poised to replace Marten de Roon, who is seemingly on the verge of a return to Atalanta BC, in Garry Monk's first-team setup at the Riverside Stadium.

Baker has spent the last two seasons on loan with Dutch outfit Vitesse, scoring 15 times in 64 appearances for the Eredivisie side.
